Falsely penalized for shipping an oversized box

So I send in a 22x22x22 box in to FBA totally secure in a 200# rated box and gummed security tape horizontally and vertically (within FBA allowance) and I get a message weeks later telling me that they’ve temporarily suspended my shipping privileges because I sent in oversized box.

I look at the pictures that Amazon provided as to the box they received and it’s not even the box I sent them. Someone along the way reboxed my shipment into an oversized box and it arrived at Amazon crushed. I’ve never had this happened before and I’m not sure how I can prove that I didn’t send it in that way but I’ll be in contact with UPS. Has this ever happened to anyone? It seems like no matter how secure or perfect I make a shipment there’s always a random chance of being penalized.
