HELP!! With Maryland Reseller’s permit / Resale Certificate

Can someone who does FBA and is in Maryland answer a few questions regarding the application for Maryland Sales and Use Tax Exemption Certificate? Don’t want to fill anything wrong and get rejected. An LLC and EIN is already in hand just not sure about these Sections and Schedules.
