Where is the appeal page?

Ok I’ve got my account got shut down because they failed to validate my information. Apparently I need to appeal and provide missing information

Cool.

But when I go to the performance notification and click on “reactivate now” I end up on that page:

There is no appeal button there:(

Anyone knows where to go?
