How do people rank for adult keywords?

Specifically, I see many products ranking in the supplements category for keywords such as sexual enhancer, strong erections… However, Amazon won’t let you use these type of words in the title, description or anywhere, not even for PPC. So how do people actually get ranked for these keywords?
