INFORM Act Tax Questionnaire

Hi sellers,

I am having a hard time trying to complete the INFORM Act Tax Questionnaire
When I write my info and click submit I am taken to another page where the status shows as pending and the 2 options I have is retake or exit.

I opened a case and the only answer is clean your cache and try a different browser, I cleaned my cache and tried Mozilla Firefox and also I tried a different computer but I keep getting the same kafkian loop,

Any idea about what Should I do? Amazon is threatening with deactivating account in a few days.
